Jeff Panzer

Jeff Panzer is a seasoned medical professional with extensive experience in care integration and innovation. Currently serving as the Medical Director of Care Integration & Innovation at Medical Home Network and as the Family Physician/Owner at Bridge Direct Care, Jeff has held various significant roles, including Physician Innovator and Director of Innovation at AllianceChicago, and Vice President of Care Transformation at Tapestry 360 Health. Prior experience also includes leadership positions at Harken Health, Iora Health, Oak Street Health, and Erie Family Health Center, where Jeff advocated for quality improvement and patient-centered care. Jeff holds a Master of Science in Healthcare Quality & Patient Safety from Northwestern University and earned a Doctor of Medicine degree from Temple University, along with a Bachelor's degree in Neuroscience from Pomona College.
