Azlan Tariq, D.O. FAAPMR CIME has a comprehensive work experience in various healthcare roles. Starting in 2008, they completed an Osteopathic Traditional Internship at Mt. Sinai Hospital. From 2009 to 2012, they served as a Resident Physician in Physical Medicine and Rehabilitation at NORTHWESTERN MEMORIAL HEALTHCARE MARIANJOY REHABILITATION CENTER AU, gaining extensive training in various areas including electromyography, ultrasound-guided injections, and sports medicine.

In 2012, Azlan joined the University of Wisconsin-Madison as a Pain and Musculoskeletal Fellow. Azlan then took on the roles of Director of Physician Development and Vice President of Medical Affairs at Integrated Rehab Consultants, leading strategic planning and overseeing clinical services.

Moving forward, they worked as a Medical Legal Consultant at Inspe Associates from 2016 to 2020. Simultaneously, they co-owned and served as the President of Optimal Health Medical Fitness, focusing on diagnosing and treating musculoskeletal and neurological conditions. At the same time, they introduced health tech into the practice.

In 2018, Azlan became a Consulting Specialist at Physician Life Care Planning, providing specialized expertise. Azlan also held the position of Chief Clinical Officer at Medrina from 2019, leading the development of clinical policies and procedures, ensuring high-quality patient care, and introducing health tech.

Azlan is a Future Leader Member of the American Academy of Physical Medicine & Rehabilitation, selected for a two-year curriculum focusing on strategic planning, media skills, advocacy, and Academy governance. Additionally, they were a Digital Health Fellow at Doximity.

Azlan Tariq, D.O. FAAPMR CIME completed their education with a Doctor degree in Osteopathic Medicine from Lake Erie College of Osteopathic Medicine in 2008. Prior to that, they earned a Bachelor of Arts degree in Business, Anthropology, and Biology from Stony Brook University from 1999 to 2002. Azlan is currently pursuing a Master of Business Administration (MBA) degree in Business Administration and Management from Gies College of Business - University of Illinois Urbana-Champaign, with an expected completion in 2026. Additionally, they hold certifications from Harvard Business School Online in Health Care Strategy and is a Fellow of the American Academy of Physical Medicine and Rehabilitation. Azlan is also a Board Certified physician in Physical Medicine and Rehabilitation. Azlan holds medical licenses from several states, including Illinois, Indiana, Michigan, Wisconsin, Oklahoma, and Ohio.
