Michelle Sommers

Michelle Sommers, LSW, has extensive experience in the field of social work and case management, currently serving as a Case Manager at Meridian Services since March 2017. Previously, Michelle worked as a Licensed Social Worker at Elim Rehab & Care Center, where responsibilities included coordinating care plan reviews, facilitating care conferences, and leading dementia training sessions. Additional roles include Patient Advocate at Cardon Outreach, Social Work Intern at the Alzheimer's Association®, and positions in community outreach, coaching, and customer service. Michelle holds a Bachelor of Social Work from Minnesota State University, Moorhead, and an Associate of Arts from Ridgewater College.
