Pahm Chelsea Hogan

Chelsea Hogan, PAHM, has extensive experience in healthcare management, currently serving as the Patient Care Services Manager at Michigan Head and Spine Institute since May 2018. Prior roles include Utilization Management Operational Lead, Care Coordinator, and Care Coordination Support Specialist at Meridian from April 2015 to May 2018, and Scheduling Coordinator at Area Agency on Aging 1-B from June 2014 to April 2015, where responsibilities included scheduling critical assessments and reassessments for program participants. Chelsea also worked as a Care Transitions Coach at the Area Agency on Aging 1-B, supporting geriatric patients during hospital discharges, and held positions as a Wireless Consultant at Verizon and a Pharmacy Technician at VG's Pharmacy. Chelsea earned a Bachelor of Science in Health Care Administration from the University of Michigan.
