Kate Sullivan

Kate Sullivan is a Senior Social Worker and Team Leader at Monash Health, with a strong background in social work across various settings. Since August 2016, Kate has held multiple roles, including Social Work Disability Specialist and Clinical Social Worker, with extensive experience in complex discharge planning, NDIS, VCAT processes, and addressing psychosocial issues in subacute inpatient rehabilitation and neuro-rehabilitation wards.
