Rachel Kwan

Rachel Kwan currently serves as the Manager of Advance Care Planning & Palliative Care at the Agency for Integrated Care in Singapore, overseeing grant management, programme administration, and community care sector engagement since May 2024. Previously, Rachel held various senior managerial roles at the National Youth Council Singapore from 2014 to 2024, where responsibilities included partnerships and engagement, grant administration, and sector development through innovative programmes and digital products aimed at youth. Rachel also pioneered a traineeship programme for youths affected by COVID-19, managed volunteer systems, and was involved in strategic outreach within the youth sector. Prior experience includes an internship at the National Council of Social Service and an Assistant Manager role at the Singapore Prison Service, focusing on community relations and rehabilitative services. Rachel holds a Master's degree in Gerontology from the Singapore University of Social Sciences and a Bachelor's degree in Sociology from the National University of Singapore.
