Care Coordination Team

The Care Coordination Team at Nascentia Health is dedicated to managing and streamlining the healthcare services for patients at home. This team, comprising Care Managers, Care Coordinators, a Team Lead, a Certified Home Health Aide, a Care Transitions Coach, and a Case Manager, works collaboratively to ensure that patients receive comprehensive, personalized care plans. They coordinate between various healthcare providers, monitor patient progress, and facilitate seamless transitions between different levels of care, ensuring that patients remain healthy and independent in their homes.
