The patient-centered medical home—one of modern healthcare’s most important innovations—is a model of care that emphasizes care coordination and communication to transform primary care into “what you want it to be.” We Are The Leaders One Community Health was the first to be recognized as a Level III Patient Centered Medical Home in Sacramento by the National Committee for Quality Assurance.
