One Community Health is a primary healthcare and specialty care provider dedicated to improving the health and well-being of our community.

Since our beginning in 1989, it has been our mission to create a healthier Sacramento by increasing access to care for all members of our community, regardless of age, gender, ethnicity, orientation, or ability to pay.

We want you to feel comfortable and well cared for. But more than that, we want you to be a partner in your care. We want you to understand, to ask questions, and to be aware that you can work in partnership with the staff to make life choices that will lead to your improved health.

We Are Your Medical Home
We use a medical home model of care, including preventive care, routine checkups, immunizations, and management of acute and chronic illnesses. The patient-centered medical home—one of modern healthcare’s most important innovations—is a model of care that emphasizes care coordination and communication to transform primary care into “what you want it to be.”

One Community Health was the first to be recognized as a Level III Patient Centered Medical Home in Sacramento by the National Committee for Quality Assurance. We vow to always be on the cutting edge of the highest quality medical care for our patients.
