Chief Performance Improvement Officer, Sara Hogue joined PCC in 2015. Sara is a passionate public health administrator that is committed to quality assurance aimed to improve population health. She has nearly a decade’s worth of experience in highly relevant performance improvement areas such as research, program evaluation, data analysis, quality assurance, and performance management. Sara received her Master’s in Social work from DePaul University and a Bachelor’s in Liberal Arts from St. Xavier University. In her previous role as Director of Performance Improvement, Sara managed the National Committee for Quality Assurance’s Patient Centered Medical Home (PCMH) recognition process, facilitated an increase from Level 2 to Level 3 PCMH recognition, and expanded recognition from four to eight sites. Further, Sara helped pioneer the organization’s Core Culture Committee, which helps identify areas to enhance inclusivity of all patients. In her role as Chief Performance Improvement Officer, Sara will lead initiatives to help develop new capacity and capabilities in quality improvement across the organization.

PCC Community Wellness Center (PCC) began in 1980 as the Parent Child Center, a three-room clinic at West Suburban Medical Center that offered prenatal, postpartum, and infant care for underserved residents of Chicago’s Austin community. In 1992, PCC was incorporated as an independent, 501(c)(3) nonprofit organization. PCC attained the status of a Federally Qualified Health Center (FQHC) look-alike in 1994 and received full FQHC recognition from the U.S. Department of Health and Human Services' Health Resources and Services Administration in 2002. Since then, PCC has grown to encompass thirteen health centers, serving West Side Chicago and the near west suburbs. PCC provides inpatient care at West Suburban Medical Center and Norwegian American Hospital.
