Personal Care Medical Associates provides physician-led geriatric and palliative medicine services to patients in skilled nursing facilities (SNF), long term care homes (LTC), assisted living facilities, personal care homes, long term acute care facilities, and hospitals throughout Western Pennsylvania. We are a team of physicians, nurse practitioners, a PharmD, nurses, and support staff whose complete focus is on providing the best care to patients in the facilities listed above. Our quality program is second to none. We also perform chronic care management and advance care planning services. We recognize the challenges of taking care of sick and elderly patients, and are a practice that puts this at the forefront, with no competing agenda. We are a patient-centric practice, who create personal treatment plans for each patient, based on not only their clinical conditions, but also their goals and values. We know that treating older adults includes involving families and care givers in decisions, and we make this an important part of our practice. We are a strong partner to health plans, hospital systems, ACO’s, and physicians. We strive to treat people in place, avoiding unnecessary hospitalizations, reduce facility length of stay, and decrease costly diagnostic tests and treatments that will not positively impact patient care. We work with our partners to ensure proper and safe transitions for our patients. We also emphasize palliative services, and are always keeping patient goals and values at the forefront of our treatment plans. It is an honor to be involved in the lives of patients at some of their most vulnerable moments, and we hope to bring compassion, top notch medical care, and peace to each life we touch.
