Our team of nurses, physical therapists, occupational therapists, respiratory therapists, speech therapists, medical social workers and home health aides will work with patients and family members to help transition patients back to their home after hospitalization, a major illness or recommendation from your physician. We’re licensed in 19 counties in Illinois. Services provided include home care services (i.e. activities of daily living), home health services (i.e. wound care, medication management, IV infusion), and other services (i.e. medical supplies coordination, home visiting doctor). Our holistic approach thoroughly understanding the patient and client status, creating a customized plan of care, continuously seeking to improve our quality of care, listening to our patients, clients, employees and third party partners, and providing a healthy workplace has contributed to our 7th consecutive growth.
