Health Navigator at Roots Community Health Center

Roots Community Health Center

Roots Community Health Center is a 501(c)3 non-profit organization dedicated to reducing health disparities in East Oakland, California. Roots is the first community health center in East Oakland to provide physician care at patient homes and residential facilities. Roots Community Health Center celebrated the opening of its new facility at 9925 International Blvd. in March of 2011.
