Ryan Health
April Evangelista is a seasoned healthcare professional with extensive experience in nursing and public health. Currently serving as the Chronic Care Nurse Manager at Ryan Health since December 2020, April previously held the position of Senior Staff Nurse at NYU Langone Medical Center from June 2014 to December 2018. Earlier in the career, April worked as a Clinical Liaison and City Research Scientist II at NYC DOHMH from November 2008 to July 2012 and as a Health Educator and Diabetes Care Manager at Montefiore Medical Center from April 2004 to November 2008. April holds a Bachelor of Science in Nursing from Arkansas State University and a Master of Public Health in Community Health Education from New York University, in addition to a Bachelor of Arts in Psychology from the University of Nevada-Las Vegas.

Ryan Community Health Network), is a Federally Qualified Health Center (FQHC) based in Manhattan that has been providing high-quality, comprehensive and affordable primary and specialty care to New York’s diverse and underserved communities since 1967.
