Enhanced Care Management Lead Care Manager/ Quality Improvement Coordinator at San Diego Family Care

Tinny Chakma has worked in a variety of roles since 2010. In 2010, they were an Honorary Medical Officer at BANGABANDHU SHEIKH MUJIB MEDICAL UNIVERSITY. In 2011, they were a Medical Officer at Anwer Khan Modern Medical College and Hospital. From 2015 to 2017, they were a Medical Interpreter- San Diego Health & Human Agency Assignments at Interpreters Unlimited, Inc. During this time, they also worked as an Enrollment Specialist at 2-1-1 San Diego and an Administrative Assistant at Bankers Life. In 2016, they were a Research Assistant at Rady Children's Hospital-San Diego. Since 2019, they have been a Quality Improvement Coordinator and Enhanced Care Management Case Manager and Mental Health and Health Homes Program Case Manager at SAN DIEGO FAMILY CARE, where they help to establish and maintain an ongoing resource list and relationship with local community resource agencies, coordinate case management of patients in conjunction with health providers, and contact patients at the provider's request to help determine the patient's status and ongoing treatment needs.

Tinny Chakma has an extensive education history. From 2003 to 2009, they obtained a Bachelor of Medicine, Bachelor of Surgery (M.B.B.S.) from Shaheed Ziaur Rahman Medical College. From 2013 to 2015, they earned a Master of Public Health (M.P.H.) from National University in Health/Health Care Administration/Management. From 2016 to 2019, they completed a Doctor of Philosophy (Ph.D.) from San Diego University for Integrative Studies, SDUIS. They are currently enrolled in Southwestern College's Nursing Program, where they are pursuing a Registered Nursing/Registered Nurse degree. Additionally, Tinny Chakma has obtained certifications from Community Health Group, Rady Children's Hospital-San Diego, UCSD-HSRC, and 2-1-1 San Diego.
