Lidia Valencia

Lidia Valencia is an experienced Community Health Worker currently employed at Santa Clara Family Health Plan since July 2021. Lidia has a longstanding association with The Health Trust, where roles have included Data Entry Clerk, Cover California Certified Counselor, Ventanilla de Salud Promotor, Program Associate, and Chronic Disease & Diabetes Self Management Program Leader. Responsibilities have encompassed outreach, enrollment, referrals, and wellness program leadership. Lidia holds a Community Health Worker Certification from Foothill College and a Diabetes Life Coach degree from Global University.
