Sally Berglin has served as the Chief Executive Officer and Interim Chief Executive Officer at Senior Care Partners P.A.C.E. since November 2021. Sally also works as a Healthcare Consultant at Codenity Inc. since September 2021. Prior to these roles, Sally was a Healthcare Project Specialist at Compliance One Group and retired as the CEO of Bronson LakeView Hospital in February 2014. Throughout their career, Sally has integrated senior services in various locations and consulted on regulatory and operational challenges within healthcare settings.

Our program coordinates and provides care for the at-risk frail elderly population of our community through an integrated team of health care professionals made up of medical doctors, physician assistants, nurse practitioners, physical therapists, occupational therapists, speech therapists, recreational therapists, dieticians, and medical social workers all under one roof. This team meets daily to assess the complex health and social needs of our participants and to make any necessary adjustments to the personalized care plan for each individual. Additional medical specialties such as audiology, dentistry, optometry, and podiatry are also offered. Our in-house day center, medical clinic and therapy rooms make it highly convenient for our participants to receive the socialization they need as well as receive comprehensive medical care under one roof.
