Healthcare and Community Services

The Healthcare and Community Services team at STRIDE Community Health Center works collaboratively to ensure comprehensive and patient-centered care. This team encompasses roles such as Care Navigator Trainers, who educate care coordinators to assist patients in navigating the healthcare system; the Deputy Director of Equity and Engagement, who promotes inclusive practices and community outreach; Community of Care Regional Managers, who oversee local implementations of healthcare services; and Certified Diabetes Care and Education Specialists, who provide specialized support and education to patients managing diabetes. Their combined efforts are dedicated to improving health outcomes and accessibility for underserved populations across multiple counties in Colorado.
