The Medical and Social Work Team at The Elizabeth Hospice provides comprehensive, interdisciplinary care to patients facing life-threatening illnesses. This team comprises Certified Home Health Aides, Medical Social Workers, a Team Medical Director, a Clinical Manager, and a Triage Patient Care Coordinator.
