Candice Rivera is a dedicated professional with a diverse background in behavioral health and customer service. Currently serving as a Behavioral Health Scheduler and Case Manager at The Floating Hospital since August 2019, Candice engages patients in treatment, assists in preparing reports on patient compliance, screens patients for program participation, and manages follow-up appointments and documentation. Previously, in roles at Fidelis Care as a Behavioral Health Associate and Member Services representative, Candice built authorizations for mental health services and handled crisis situations. Initial experience as a Customer Service Representative at Time Warner Cable and a Travel Agent at Global Contact Services further honed communication and organizational skills. Candice holds a Bachelor of Arts in English Language and Literature from The City College of New York.

The Floating Hospital

The Floating Hospital is the only Federally Qualified Health Center (FQHC) in New York City to have all three of the federal government’s urban FQHC designations: to provide medical care to the community, to public housing residents, and to the homeless. The Floating Hospital is also a certified Level 3 Patient Centered Medical Home. As family doctor to New York City’s most vulnerable citizens, The Floating Hospital has continuously provided primary healthcare to at-risk populations in all five boroughs of New York City for 148 years in fulfillment of its mission: to provide high-quality, comprehensive, compassionate healthcare and social services to underserved communities. Since the tragic events of 9/11, we are no longer “floating” but are now land-based in our main Long Island City clinic, in six satellites in New York City homeless shelters, and in our new Queensbridge clinic. From our state-of-the-art Long Island City facility and satellite locations across all five boroughs of New York City, we continue our pioneering tradition of providing New York City’s underserved populations with the consistent, respectful healthcare New Yorkers expect from their personal family physicians. We provide primary medical, dental, and mental healthcare as well as health education and counseling. Our bilingual and culturally competent clinical teams ensure that each patient is accurately assessed and treated with dignity and compassion. By offering free van transportation to and from our facilities, we help our patients overcome a major barrier to healthcare regardless of their ability to pay for services.
