Carl Czuboka

Carl Czuboka is an experienced adult medicine physician currently serving at The Southeast Permanente Medical Group since July 2023. Previously, Carl Czuboka held various leadership positions at Kaiser Permanente from July 2007 to July 2023, including Associate Medical Director for Primary Care and Behavioral Health, Chief of Staff and Associate Medical Director for Business Systems & Strategy, and Chief of Adult Primary Care. Prior to this, Carl Czuboka worked as a Staff Physician at Alcan from March 2003 to May 2007 and at CSSS de la Montagne from 1997 to 2007, providing diverse medical services. Educational qualifications include a Doctor of Medicine from the University of Ottawa, obtained in 1990, a Family Medicine Residency Program at McGill University, and a Bachelor of Science in Biochemistry from the University of Ottawa.
