Jacob Diogostine

Jacob Diogostine is an experienced professional in the health and social services sectors, currently serving as the Lead of Care Navigations at Springtide Child Development since February 2022, where the Care Navigations Department was built from the ground up. Prior to this role, Jacob Diogostine worked at Cityblock Health from February 2019 to October 2021, initially as a Community Health Partner Lead, providing training and support for engagement and care planning, and then as a Social Care Lead, supervising a team of Community Health Partners to enhance patient health through outreach and comprehensive assessments. Earlier experience includes serving as a Program Manager at New Opportunities, Inc., where Jacob Diogostine developed an employment pilot program called “Secure Jobs” in Connecticut. Educational qualifications include a Bachelor's degree in Criminology/Psychology from Central Connecticut State University and a Bachelor's degree in Criminal Justice from the University of New Haven.
