Nancy Rose, LSW, has over three decades of experience in social work, specializing in the care and support of various populations, including the elderly and individuals facing social challenges. Currently serving as a Social Worker at Trinity Health PACE since June 2016, Nancy provides education, advocacy, and assistance to participants aged 55 and older, focusing on aging in place and community resource access. Previous roles include Case Management at Baystate Health, where Nancy addressed challenges such as substance abuse and homelessness, and a Rehab Social Worker position at CareOne Management LLC, where success in streamlining discharge processes was achieved. Educationally, Nancy holds a Bachelor's degree in Psychology from Westfield State University.
