Samuel DeLeon, MD, is the Senior Vice President Medical Affairs/Chief Medical Officer of Urban Health Plan (UHP). He received his Bachelor of Science from the University of the State of New York. He completed medical school training in the Dominican Republic and the University of Puerto Rico Medical School. He performed his internship at St. Francis Hospital in Pittsburgh, a residency in internal medicine at New Rochelle Hospital in NY and a fellowship in Pulmonary Diseases and Sleep Medicine at New York Medical College.

Dr. DeLeon joined Urban Health Plan in 1994 as a physician and rose to become section head of adult medicine, medical director and chief medical officer. He was instrumental in overseeing UHP’s adoption of health information technology, supported a mentorship program to foster clinical retention and developed a successful pay for performance program. Valuing continuous quality improvement, he co-founded the Institute for the Advancement of Community Health, created a Best Practices committee and guided a Community Research Unit through clinical trials.

Dr. DeLeon is a diplomate of the American Board of Internal Medicine and the American Board of Medical Management. He is also a member of the American College of Physicians, the American College of Physician Executives, the American Thoracic Society, the Bronx County Medical Society, the Clinical Directors Network Internal Review Board and the National Hispanic Medical Association. In 2016, he was named Clinician of the Year by the Association of Clinicians for the Underserved.
