Doctoral Student Saffiyyah Muhammad Ma

Saffiyyah Muhammad, MA, is a Doctoral Student with extensive experience in healthcare coordination and research. Currently serving as a Transition of Care Care Coordinator at Access Community Health Network since 2013, Saffiyyah focuses on ensuring effective discharge planning and smooth transitions for patients returning to their medical homes. Previous roles include Maternal Child Health Care Coordinator, where Saffiyyah managed high-risk pregnancies among diverse populations, and Research Assistant at the University of Illinois at Chicago, implementing research aimed at promoting adolescent health. Saffiyyah's background also includes providing psychotherapy as an Intern Psychotherapist at CommunityHealth and advocating for patients at the University of Chicago Medical Center.
