Rich Porcelli

Director, Community Engagement at Activate Care

Rich Porcelli is the Director of Community Engagement at Activate Care. Rich has dedicated their career to improving population health and addressing social determinants of health.

Rich has over 7 years of experience working with healthcare organizations to implement population-level approaches to addressing social determinants of health. Rich is a skilled coach, trainer and facilitator, and has a deep understanding of healthcare financing, cross-functional team management, and complex project management. Rich utilizes Human Centered Design methodology and IHI's quality improvement methodology to drive sustainable change initiatives.

Rich's work has extended to a variety of system settings, including ACOs, MCOs, PCMH, mental health and rural health clinics. At a community level, they have led a health and housing regional cross-sector project focused on bringing together hospital systems, public health departments, community-based organizations and funders to address health equity through housing.

Rich has deep experience designing and facilitating peer-based learning initiatives including; communities of practice, improvement collaboratives, virtual learning workshops and networks. Working in a variety of roles they have been a consultant, coach and trainer supporting healthcare systems undergoing transformation to new models of payment and performance.

Nationally recognized as a subject matter expert on social determinants of health, Rich has been privileged to present at conferences and as learning faculty across the country at conference for American Academy of Pediatrics, Center for Medicare and Medicaid Innovation Center and Massachusetts department of public health.

Rich Porcelli has a Masters in Organizational and Counseling Psychology from Adler University and a Bachelors of Arts in Psychology from Providence College.

Their manager is Ted Quinn, CEO & Co-Founder. They work with Katie Hurley - VP, Product, Pete Agrawal - Director, Technical Product Management, and Josh Winslow - VP, Engineering.
