ALONE provides direct, frontline services to people aged 60 and over.  ALONE provides an integrated system of support coordination, practical supports, support and befriending along with a variety of Phone Services, Social Prescribing, Housing with Support, and assistive technology. 

ALONE implements support plans, provides a point of contact for access to health, social care, housing, transport, and other emerging needs through the utilization of technology and various services and activities. These efforts aim to enhance overall physical, emotional, and mental wellbeing.

We are now recruiting a for a Support Coordinator in South Cork City.

The successful candidate, upon commencing employment, will undergo a comprehensive induction and training programme to ensure readiness for the role's responsibilities and will have the opportunity to make a positive impact on the lives of numerous older people in Ireland.

Support Coordinators play a critical role in facilitating practical support and coordinating services for older people to address their challenges effectively. The role focuses on assessing needs, connecting with partners, and coordinating services to ensure a comprehensive and targeted approach.

Support Coordinators are field based, and travel is required within the community. A clean driver’s license and own car is mandatory.

Responsibilities

  • Provide support coordination by devising personalised support plans to address challenges and find solutions to enhance the older persons quality of life.
  • Provide access to ALONE’s services while coordinating and enabling older people to access other services in the community.
  • Advocate for older people's needs within health, social care, and welfare services, working to streamline access and enhance service delivery.
  • Establish and nurture strong referral relationships with both statutory and non-statutory agencies in the designated area, fostering collaboration and access to a wider range of support services.
  • Develop and maintain efficient referral pathways for ALONE’s Support Coordination services, optimising service coordination.
  • Adherence to ALONE's Quality management systems and policies, ensuring a consistent and high-quality service delivery approach.
  • Monitor and manage key performance indicators (KPIs), striving to achieve SMART targets that maximize the reach and impact of support services for older people.
  • Deliver thorough and timely reports on service developments, challenges, and achievements.
  • Manage the distribution and installation of technology that enhances older people's well-being and safety.
  • Maintain records on ALONE’s CRM database, capturing essential details to inform service delivery and future strategies.

Essential skills & experience

The successful candidate will be able to demonstrate:

  • Demonstrated passion for improving the lives of older people and commitment to ALONE’s mission.
  • Problem-solving skills, focusing on efficient needs assessment and resource allocation.
  • Excellent communication abilities (both written and verbal), fostering partnerships and negotiating effectively with diverse stakeholders.
  • Proficiency in utilising technology and data management systems, enhancing service efficiency and reporting accuracy.
  • Good organisational and multitasking skills, with a solution-focused approach to complex challenges.
  • Keen attention to detail, enabling accurate record-keeping and effective service monitoring.
  • Enthusiasm for setting and achieving SMART targets that maximize the positive impact on older individuals.
  • Self-starter with a capacity to work well with stakeholders
  • Must have full clean driving licence and own transport.

 A background in older people-related fields is not required.The key factor is a strong alignment with ALONE's mission and the ability to efficiently coordinate services and address challenges faced by older people.
