Elizabeth Holbert

Elizabeth Holbert, LMSW, currently serves as a Hospice Social Worker at Amedisys since July 2023. Previous experience includes roles as a Hospital Care Manager-Social Worker at Prisma Health and as a Hospice Social Worker at Amedisys, where duties involved direct patient care, resource identification, and education on Hospice and Palliative Care. Elizabeth also worked at Kindred as a Hospice Social Worker/Bereavement Coordinator, assisting terminally ill patients and families with psychosocial needs and coordinating care plans.
