Tanía J. Stack

Tanía J. Stack, LCSW, CHC, is an experienced Behavioral Health Specialist currently working at Oak Street Health since March 2024, where responsibilities include collaborating with primary care teams and facilitating patient referrals to the Behavioral Health program.
