Matthew Hazelbaker M.d.

Matthew Hazelbaker M.D. serves as the Chairman and President of Avina Women's Care since January 2021 and has been the Credentials Chair at Memorial Health Ohio since January 2017, overseeing hospital privileges and peer reviews for physicians. Previously, Matthew held various leadership roles including President of Marysville Obstetrics & Gynecology (2010 - January 2021), Medical Staff President at Memorial Hospital of Union County (2011 - 2017), and Chairman of the Department of Surgery at Memorial Hospital of Union County (2007 - 2010). Additionally, Matthew was the Chairman of the Board of Managers at Marysville Ohio Surgery Center (2007 - 2008). Matthew completed an Obstetrics & Gynecology residency at The Ohio State University Wexner Medical Center (1995 - 1999) and earned a Doctor of Medicine degree from The Ohio State University College of Medicine (1991 - 1995) after obtaining a Bachelor of Arts in Pre-Medicine from Miami University (1987 - 1991).
