POSITION PURPOSE: 

Under the supervision of a Registered Nurse (RN), Licensed Independent Practitioner (LIP) or the Nurse Manager, the Licensed Practical Nurse (LPN) performs clinical duties within their scope of practice.  The LPN will be responsible for planning, assigning, coordinating and providing nursing care and services for patients and function as a resource to the Medical Assistant staff.  The LPN will assist in the development of systems that maximize patient service, safety, quality care and provider efficiency.  In addition, the LPN will assist in Department Quality Assurance and Performance Improvement activities under the direction of the RN and/or Nurse Manager.

Standards related to the LPN’s responsibility for ethical practice, accountability for services provided, and competency. The LPN shall:

  • Base their practice on current nursing science, other sciences, and the humanities;

  • Be knowledgeable of the statutes and regulations governing LPN practice in Oregon and practice within those legal boundaries;

  • Be knowledgeable of the professional nursing practice standards applicable to LPN practice and adhere to those standards;

  • Maintain competency in one’s LPN practice role;

  • Accept only LPN assignments that are within one’s individual scope of practice;

  • Protect confidential client information and only share information in a manner that is consistent with current law.

ESSENTIAL FUNCTIONS:

At the clinical direction and under the clinical supervision of the RN, LIP or Nurse Manager:

  • The LPN may participate in the development, implementation and evaluation of teaching plans appropriate to the context of care, that address the patient’s learning needs, readiness to learn, and ability to learn.

  • Conduct focused assessments by collecting data through observations, examinations, interviews, and records in an accurate and timely manner as appropriate to the client's health care needs and context of care.

  • Maintain competency in the LPN practice role.

  • Maintain documentation of the method that competency was acquired and maintained.

  • With documented competency the LPN will administer injections and screens as ordered by providers.

  • Distinguish abnormal from normal data, sort, select, record, and report the data discrepancies to the supervising RN or supervising LIP. Inform patients of abnormal lab test and x-ray results.

  • Provide support and education of Medical Assistants that includes mentoring, training and oversight in basic skills and procedures, testing knowledge base and giving in-services.

  • Participate in implementation of clinical programs for patients with chronic conditions.

  • Maintains good attendance, is punctual and works full scheduled shift is a condition of employment.

  • Demonstrates respectful, professional and appropriate behavior that supports a team oriented work environment.

  • Check in and room patients and schedule patient appointments as needed.

  • Attend staff meetings, in-service meetings and participate in agency committees and task force activities as required.

  • Perform other duties as assigned by supervisor.

QUALIFICATIONS: 

  • Must have an active and unencumbered Oregon Practical Nurse license.

  • Experience in an ambulatory setting preferred. (New graduates will be considered.)

  • Experience with EMR preferred.

  • Ability to work independently, detail oriented, organized, work effectively under pressure, and meet deadlines

  • Excellent computer and organizational skills

  • Excellent written and verbal communication skills

PHYSICAL REQUIREMENTS: 

  • Standing, walking, sitting, lifting and able to carry up to 25 pounds.
  • Pushing/pulling, bending, stooping, twisting, reaching and handling, climbing stairs (Elevator available).
  • Hearing: within normal limits with or without use of corrective hearing devices.
  • Vision: adequate to read 12-point type with or without use of corrective lenses.
  • Must be able to verbally interact with staff, clients and public.
  • Manual dexterity of hands/fingers for writing, computer input.
