Jaime Fazzone

Jaime Fazzone, PharmD, BCPP, currently serves as a Clinical Psychiatric Pharmacist at AxessPointe Community Health Centers since July 2024. Prior to this role, Jaime held the position of Chair of the Behavioral Health Special Interest Group at the Washington State Pharmacy Association from November 2021 to July 2024 and worked as a Clinical Behavioral Health Pharmacist at CHAS Health from June 2021 to July 2024. Jaime's experience includes serving as a Clinical Pharmacist at Eastern State Hospital from December 2018 to May 2021 and at Providence Health & Services for a brief period in 2018, where responsibilities encompassed managing medications for chronic diseases and providing drug information to healthcare providers. Jaime’s foundational training includes completing a Doctor of Pharmacy at the University of Colorado Anschutz Medical Campus and a Bachelor of Science in Pharmaceutical Sciences at The Ohio State University.
