Sr. Director Complex Care Management at Boston Medical Center

Erin Nahrgang, RN, CCM, has extensive experience in healthcare management and innovation. Currently serving as the Sr. Director of Complex Care Management at Boston Medical Center since February 2018, Erin has held various leadership roles including Director of ACO Complex Care Management and Sr. Nurse Manager of ACO Transformation & Complex Care Management. Prior experience includes working as a Heart Failure RN at Oregon Health and Science University and as a Transitional Care and Outreach RN at CareOregon, where significant contributions were made to developing new nursing roles aimed at reducing hospital readmissions for high-risk Medicaid and Medicare members. Erin also played a key role in establishing Utilization Management and Care Management departments for Oregon's Health CO-OP. Educational qualifications include a Bachelor of Science in Nursing from Oregon Health & Science University and a Bachelor of Arts in International Studies focused on Health and Development from the University of Denver.
