Bree Whitmore

Bree Whitmore, LMSW, CCM, is an experienced social work professional with over 25 years in various roles within healthcare and social services. Currently serving as a Social Work Care Manager at Baylor Scott & White Health since April 2019, Bree focuses on telephonic care management aimed at preventing hospital readmissions. Bree’s diverse background includes roles as a Hospice Social Worker at multiple organizations, a Medical Social Worker at Harbor Hospice, and a Family Group Decision Making Specialist with the Texas Department of Family and Protective Services. Bree holds a Master of Social Work from Grand Valley State University and a Bachelor’s degree in Public Relations from Western Michigan University.
