Jan Johnson

Jan Johnson, RN, CCM, currently serves as the Transition Care Nurse Manager at CarDon and Associates since July 2015, focusing on collaboration with physician groups and advocating for patients and families to reduce hospital readmissions. Prior experience includes a tenure as Director of Case Management at Community Hospital from September 1976 to October 2008, where Jan managed a team of 14 registered nurses and social workers responsible for Utilization Review and Discharge Planning for both outpatient and inpatient care. Jan received an Associate's degree in Nursing from the University of Evansville between 1973 and 1976.
