Heidi Blake

Dr. Heidi Blake joined the Care Dimensions team in August of 2017 as a full-time hospice and palliative care physician. She is focused on managing the care of hospice patients and providing palliative care consultations for pain management and quality of life issues for patients with advanced illness.

Prior to joining Care Dimensions full time, she worked part-time at its Kaplan Family Hospice House. From 2010-2016, Dr. Blake practiced palliative medicine at Beth Israel Deaconess Medical Center on its inpatient palliative care consult service.

Board certified in internal medicine and hospice and palliative care medicine, Dr. Blake has been an instructor in medicine at Harvard Medical School. She has a fellowship in Palliative Medicine from Harvard Medical School and is a graduate of Tufts University School of Medicine.
