Nurse Practitioner Ramping Team Lead

The Ramping Team Lead for CareBridge is a vital member of the clinical team and is responsible for the ongoing execution, clinical oversight, and outcomes of the CareBridge 24/7 Member Support Program. The CareBridge 24/7 Member Support Program uses an interdisciplinary team to provide patient outreach, early detection of acute changes in condition or chronic disease exacerbations, and social determinants of health impacting care and interventions to improve patient outcomes.  The population served are patients who receive Home and Community-Based Services (HCBS).  The Ramping Team Lead supports the interdisciplinary team that works closely with patients, paid and family caregivers, health plan care coordinators, and other community providers.  

  • Actively engage in the interviewing and hiring of the team
  • Maintain oversight of Ramping staff daily schedules and facilities optimization of patient-facing hours
  • Train the Ramping team members in the clinical model, role responsibilities, CareBridge policies and procedures
  • Promote a strong, high-functioning, interdisciplinary team
  • Reports to the Ramping Team Manager and works closely with other clinical leadership to continue to develop, enhance, and further evolve the 24/7 Member Support Program
  • Participate in the development of documentation templates, clinical guidelines, care plan guidelines and templates, and workflows
  • Provide clinical and operational oversight for the Ramping team under the direction of the Ramping Manager, delivering services in the 24/7 Member Support Program
  • Responsible for both clinical and operational outcomes
  • Communicate effectively with the entire team
  • Participate in clinical case conferences
  • Lead quality improvement reviews and initiatives
  • Deliver proactive, high-quality customer service
  • Problem-solve and course-correct when appropriate
  • Will be a "player-coach", maintaining some patient-facing hours depending on the need
  • Maintains excellent punctuality and attendance during work hours, hours may vary and will include supervisor on-call rotations
  • This role may oversee direct reports
  • Other duties as assigned.
  • Five + years of clinical practice experience, previous people leadership experience preferred
  • Holds active, unencumbered Nurse Practitioner license in states of practice
  • Expertise in HCC, Coding, and RAF scoring
  • Familiarity with HEDIS measures and affordability programs a plus
  • Experience in the care of adult, chronically ill patients
  • Advanced computer skills, including experience with Electronic Medical Records and Microsoft Office Suite
  • Expert communication skills, oral and written
  • Familiarity in data analytics
  • Prior telemedicine experience a plus
  • Value-based-care and growth-stage startup experience strongly preferred
