Vivian Chau, PharmD, BCACP, currently serves as an Ambulatory Care Clinical Coordinator at CareOregon, focusing on medication reconciliation and care coordination for recently discharged patients. Previously, Vivian worked as a Population Health Clinical Pharmacist at Saint Alphonsus, where efforts concentrated on reducing hospital readmissions and improving medication adherence. Vivian’s background also includes leadership as Chief Operating Officer at LitKit, contributing to health literacy through innovative educational software, and extensive residency training in ambulatory care and academia at Pacific University. Additionally, Vivian gained valuable experience through clinical pharmacy roles and internships across multiple healthcare settings. Vivian holds a Pharm.D. from Pacific University and a B.S. in BioHealth Sciences from Oregon State University.
