Individuals and families facing serious illness or the end of life are in the best hands with Care Partners, one of two remaining independent nonprofit providers of these services in greater Portland.  Every day we deliver on our mission to provide exceptional care without exception. 

How do we do this?

•	Personalized care.  We listen to and are responsive to individual needs.
•	More health care.  We offer unique focused care programs tailored to manage the four most common serious illness diagnoses: cancer, heart failure, chronic lung disease and dementia.
•	Psychosocial care.  Dealing with life-limiting illness is one of the most stressful experiences not only for the person diagnosed, but also for their family and friends.  We take the time to provide compassionate support that transforms

•	Care for all members of our community.  Our services are tailored according to the background and identities of those we serve.  We also help those without insurance coverage with medical costs and with rent, utilities and food as required.
