Rachel Chisholm

Rachel Chisholm is an experienced social worker and administrative fellow currently working at CHEO in the Strategy, System & Quality Improvement department, starting in October 2023. Prior to this, Rachel spent over eight years at Bruyere Continuing Care, where responsibilities included collaborating with interprofessional teams on the Geriatric Rehabilitation Unit to address psychosocial impacts of hospitalization and implement patient-centered discharge plans. Rachel has also held positions at The Hospital for Sick Children as a social work intern, focusing on chronic illness and family support, and at Bridgepoint Active Healthcare, where case management and community resource connection were key roles. Academic qualifications include a Master’s Degree in Social Work from the University of Toronto and a Bachelor of Arts with First Class Honours in History from the University of King's College.
