Jill Modi

Jill Modi, PharmD, BCPS, currently serves as an Ambulatory Care Clinical Pharmacist at Cityblock Health, focusing on population health and quality improvement initiatives for Medicare and Medicaid members with diabetes. Responsibilities include developing a pharmacist-managed registry, standardizing clinical interventions for uncontrolled diabetes, executing collaborative practice agreements, and conducting telemedicine visits to provide medication counseling. Previous experience includes a Health Systems Level II Clinical Pharmacist role at New York City Health and Hospitals Corporation, where medication therapy recommendations and management were provided for internal medicine teams, as well as a pharmacy residency at Penn Medicine, which encompassed various practice areas. Early career roles included a pharmacy intern at Kaiser Permanente and a pharmacy technician at CVS Health, complemented by a research assistantship at the University of Maryland Baltimore. Jill Modi holds a Doctorate of Pharmacy from the University of Maryland School of Pharmacy and a Bachelor of Science in Biology from The University of Georgia.
