Tara Sherblom

Tara Sherblom is an experienced professional in the health and social services field, currently serving as a Care Team Manager at Cityblock Health since February 2020, after progressing through roles including Care Team Lead and Senior Community Health Partner. Prior to Cityblock Health, Tara held various positions such as Customer Service Representative at Skyline Flight, Resident Services Coordinator at Federal Management, Family Counselor at Youth Villages, and Case Manager at Elder Services of Worcester Area, Inc. Tara earned a Master of Arts in Health Advocacy from Assumption University in May 2023 and previously obtained a Bachelor of Arts in Social Work from Anna Maria College in 2007.
