Position Title: Enhanced Care Program Management (ECM) Manager

Department:  Care Management Services

Position Reports to: Mental Health Services Director

Position Summary

The Enhanced Care Management Program (ECM) Manager is responsible for overseeing all aspects of the ECM department, ensuring the effective delivery of specialized care services to individuals with complex medical conditions. This is a pivotal role in developing and implementing strategies to optimize the quality of care, improve patient outcomes, and enhance the overall patient experience.

Key Responsibilities

Program Development and Implementation:

Develops and implements effective integration of current organizational case management efforts with Enhanced Care Management Program, under Care Management Services Department.

  • Conduct comprehensive assessments to evaluate the medical, social, and psychological needs of patients.
  • Develop and implement comprehensive and personalized ECM programs and care plans in collaboration with providers, patients, and caregivers..
  • Collaborate with interdisciplinary teams to design and implement care plans that address medical, social, and emotional needs.
  • Implement initiatives to enhance program efficiency, effectiveness, and overall patient satisfaction.

Coordination and Team Leadership

  • Coordinate and manage overall care plans for individual with complex medical needs.
  • Maintain accurate and updated documentation of patience assessments, care plans, and interventions.
  • In collaboration with workforce development department, develops and implements training modules to standardize best practice among care management personnel.
  • Provide leadership and direction to ECM staff, including nurses, medical assistants, case managers, caregivers, clinical and support personnel.
  • Foster a collaborative and supportive work environment that promotes teamwork, professionalism, and excellence in patient care.
  • Facilitate communication among healthcare team members, patients, and caregivers to ensure continuity of care.

Quality Assurance and Compliance:

  • Ensure compliance with all relevant regulations, standards, and policies governing ECM services.
  • Conduct regular assessment of ECM programs and services including monitoring patient progress and outcomes through data tracking and analysis.
  • Continuously implement quality assurance measures to evaluate the effectiveness of ECM programs and services and identify opportunities for improving care delivery and outcomes.

Patient Advocacy and Support:

  • Serve as a primary point of contact for patients and their families, advocating for their rights, needs, and preferences throughout the care process.

  • Provide guidance and support to patients and families navigating complex healthcare systems and resources.

  • Connect patients with appropriate healthcare services, community resources, and support programs.

  • Provide support and intervention during medical emergencies or crises.

Stakeholder Engagement and Collaboration:

  • Establish and maintain effective partnerships with internal stakeholders, including clinical teams, administration, and support staff.

  • Collaborate with external partners, such as community organizations and healthcare providers, to coordinate care and support services for ECM patients.

Qualifications

  • Bachelor’s degree in Healthcare Administration, or related field; Master's degree preferred.
  • Minimum of 5 years of experience in healthcare management, with a focus on complex care or care coordination of vulnerable populations.
  • Demonstrated ability to lead teams effectively by setting clear goals, providing guidance, and fostering a collaborative and supportive work environment that inspires and motivates team members.
  • Excellent communication and interpersonal skills, with the ability to convey complex information clearly and effectively to diverse audiences, including health professionals, patients, and stakeholders. .
  • Proven ability to develop and implement innovative programs and initiatives.
  • Knowledge of regulatory requirements and best practices in healthcare management.
  • Demonstrates professionalism and sound judgment in directing operations and handling sensitive and confidential issues.
