Colonial Poplin is a skilled rehab facility that can support all of your respiratory needs.   Our goal is to provide short term interdisciplinary rehabilitation for patients with a primary pulmonary or cardiac diagnosis and with acute conditions affecting their functional abilities.  Patients are taught self-management strategies enabling them to reach their highest functional independence and prevent re-hospitalizations, often exceeding their prior level of function in a safe and effective manner.  In addition to our comprehensive rehab, our cardio pulmonary program includes:  Medical Director and Nurse Practitioner visits 4-5 times per week. On-sire Respiratory Therapist performing weekly rounds, Extensive resident and family education, Individualized treatment plans, Oxygen Therapy, C-PAP/BI-PAP, Aerosolized medication management, Registered Dietician, Secretion Mobilization & Airway Clearance, Chest tube care, Diagnostic testing.
