Our Community: CHCN represents eight community health centers in Alameda and Contra Costa counties in 80+ locations with 450 primary care providers and 1,100 specialists, bringing 141,000+ health plan members the best care at the right time. Our History: CHCN was formed by our member health centers in 1996 to support health centers in a rapidly expanding managed care environment. Its sister organization, Alameda Health Consortium, was founded by community health centers in 1974 to support advocacy, health insurance enrollment, and serve coordination for uninsured and low-income communities.
