Daniel Napolitano

Chief of Population Health Officer at Community Healthcare Network

Daniel Napolitano, M.D. is Community Healthcare Network’s first Chief Population Health Officer. He holds a medical degree from Rutgers New Jersey Medical School and completed his residency at the Mount Sinai Downtown Residency in Urban Family Medicine.

Dr. Napolitano served at CHN until 2016 as a family physician and Deputy Chief Medical Officer. Since then, he worked at the Institute for Family Health and Mount Sinai Hospital System as Vice President of Population Health for the Institute and Director of Inpatient Services for Family Medicine at Mount Sinai Hospital.
