The Health and Medical Programs team at Community Healthcare Network (CHN) oversees and coordinates diverse healthcare initiatives and services. They manage infectious diseases, LGBTQ programs, behavioral health, social services, clinical operations, and community health efforts. The team consists of various experts and directors who ensure the seamless delivery of patient care, facilitate care coordination, drive population health strategies, and support specialized programs like the NP Fellowship and Criminal Justice Program.
