Physician Liaison at Community Memorial Health System

racey Shoop has been a resident of Ventura for 16 years and has lived and worked in Southern California for over 27 years. She graduated from the University of Kentucky with a degree in Business Management and Economics and has worked in the Home Health and Hospice fields for over 10 years. She also worked with many of our local hospitals, integrating programs to help reduce hospital readmission rates, as well as programs within Cottage Hospital Health System, Cedars-Sinai Medical Center, UCLA, and Keck USC. Tracey’s goal is to be an invaluable asset to support the medical staff and to work with their office personnel, ensuring continued growth for CMHS.

Physician Liaisons are responsible for continually addressing the needs of our physicians and their staff by facilitating physician team relations and developing and implementing communication to help foster positive relations. This effort promotes a dynamic relationship between the hospital and the medical staff. The information shared with the Physician Liaison is highly valued and is used by the Hospital in planning and evaluating services and programs. The Physician Liaison can serve as a single point of communication for your practice when questions, concerns, or issues arise, in addition to a two-way communication between community physicians, office staff, and Community Memorial Health System.
