Clinical Support Team

The Clinical Support Team at Concert Health is dedicated to enhancing the provision of behavioral health care within primary care settings. Composed of Collaborative Care Clinicians and Behavioral Health Care Managers, this team collaboratively assess and manage patients experiencing depression and anxiety, ensuring they receive timely and effective treatment. By leveraging their expertise, they facilitate seamless communication between primary care providers and mental health professionals, ultimately aiming to improve patient outcomes and support the implementation of evidence-based care models like Collaborative Care Management (CoCM).
