We founded Crescent in response to personal healthcare experiences and the perceived need in South Carolina for a better hospice option for patients and their families. The Clinical Care Team at Crescent Hospice is dedicated to providing compassionate, patient-centered care to individuals facing serious illnesses and their families. This multidisciplinary team, comprising registered nurses, case managers, physiotherapists, and hospice specialists, collaboratively develops and implements personalized care plans, ensuring that each patient's medical, emotional, and psychological needs are met with dignity and respect. Their goal is to enhance the quality of life for patients and provide support to families throughout the hospice journey.
